How to apply:

The eligible and willing candidates may submit their application form to the office of the

Medical Superintendent ESIC Hospital Baddi for walk-in-interview along with duly filled in application

form would be provided at the time of interview. The candidate shall come along with original

certificates arnd one set of self-attested photocopies of the relevant certificates/documents in support of

Age, Qualification, Category, Experience, MCl/ State Registration and two recent passport size

photographs at the time of interview

Document required: -

T Age proof.

2 MBBS certificate.

3 Two photographs. (PP size)

4 Caste Certificates for SC/ST/OBC Candidates. (OBC Candidates are required to submit latest

OBC Certificate as per Central Govt. Proforma (not more than one year old) from the date of

intervi

S PG Degree/Diploma Certificate.

6 State Medical Council/MCI Registration Certificate with MS/MD/DNB qualification.

7 No Objection Certificate from present employer, if applicable.

8 Experience Certificate if applicable.

9 Income and asset certificate from issuing authority as per DOPT letter N

36039/1/2019- ESTT. (Res) dated 3 1/01/2019 must be submitted for the reservation for EWS

Category.

Terms & Conditions: O1. No TA/DA shall be paid for attending the interview and joining purpose

Incase of selection the candidate shall have to pay Rs. 10000/-(Rupces Ten Thoursand) only as

Cposit through Demand Drall in favour of ESIC Fund Account No. I payable at 5adds the time of joining. which is refundable afer successful complction of contract period and on produiction of "No dues certilicate". . case of selection the candidate shall have to enter into as agreement with the Medical uperintendent, ESIC Hospital, Baddi on a stamp paper (Non-Judicial) of Rs. 100- The Stamp Paper will be borne by the candidate as a lirst pairty. ne competent authority reserves the right to fill up all or not to fill up any post without assigning any reason thercol. 0. vacancies are likely to change depending upon actual requirement at the time of intcrview. 06. No claim for any service benefit like PE, Pension, Gratuity, Medical Allowance, Seniority, Promotion and leave will be admissible. 2. Hostel Accommodation Quarters will not be provided. 13. he decision ot the nterview Board will be final on all aspects of selection and no further correspondence will be entertained under any circumstances. 14. The selection is subject to the candidates proving their medical fitness and verification of their character && antecedents. 15. Absence from work (duty) for a period of 15 (fifteen) days without proper permission of the competent authority will amount to voluntary abandonment of contractual-engagement and no further order of dis- cngagement shall be necessary 0. Any groSs misconduct or negligence in the assigned duty may lead to termination of contract. 17. The part time specialist will not be entitled for any kind of leave. 8. The working hour 1or the part time specialist will be four days a week and four houra day. 19.The principal of No work no pay will be applicable. 20. Candidate claiming reservation/age relaxation on ground of belonging to the OBC should submit the community certificate as prescribed by GOL. The OBC certificate should be latest and not prior to one year from the date of interview. 21. Candidates suffering from 40%% or more of the relevant disability shall only be eligible for the reservation for PH category. Certificate of disability from any Got. hospital should be submitted at the time of interview. Note: 1. Candidates who have already completed 03 years Senior Residency need not apply under Senior Residency Scheme.
